First you'll need to apply for the provisional Cat D licence. You do this by filling in form D2 and sending your licence away requesting the additional licence, along with a medical (D4 Form). Once this is back book the theory tests. They are hazard perception, multiple choice and if you're needing CPC too, the CPC case study theory test.
